Health History Immunization Form FAQs

Please read the Health Immunization form carefully and review the FAQs below so your form is approved the first time you submit it.

Q. What vaccinations are required?*

  • MMR: 2 doses, or blood titers that prove immunity
  • Varicella: 2 doses, or blood titers that prove immunity
  • Tdap: 1 dose, must be dated within the last 10 years.
  • MCV**: Most will have 2 doses. 1 dose is required after the age of 16. If you've only had one dose, it must have been received after the age of 16 to be a valid vaccination.

    *As of August 1, 2023, the COVID vaccine is not required. William Jewell College encourages all members of the community to follow CDC guidelines regarding the COVID-19 vaccine and other immunizations.
    **For non-traditional students who do not live on campus or communally, the MCV vaccine is optional, but you must note an exemption on the form in the appropriate space.

Q. What do I do if I can’t find my immunization records?
A. If you are having difficulty finding your vaccination records, the CDC recommends the following: 

  • Check with your high school for dates of any immunizations. Keep in mind that generally records are kept only for 1-2 years after students leave the system.
  • Check with previous employers (including the military) that may have required immunizations.
  • Check with your doctor or public health clinic. Keep in mind that vaccination records are maintained at doctor’s office for a limited number of years.
  • Contact your state’s health department. Some states have registries (Immunization Information Systems) that include adult vaccines.

Q. I’m a returning student and already have my immunization record on file with Jewell’s Health Center so do I need to do anything?
A. Yes!

  • You should be able to access your previously submitted records in eTrieve to assist you in the completion of a new Health Immunization Form. Please carefully review the requirements to ensure you are compliant and up to date.
  • Please DO NOT send your immunization record to the Health Center. You must upload it into eTrieve for it to be reviewed.
  • Athletes: Check your ARMS profile to obtain vaccination records so that they are up to date.

Q. I had all my vaccinations and dates listed on the form but it was still returned. What happened?
A. If you aren’t uploading your official vaccination record from your physician or your public health clinic, you need to make sure your primary care provider has signed your Health Immunization Form attesting that the vaccinations and dates you provide on the form are accurate as listed.

Please double check that you are submitting vaccination records for all required vaccines that match the dates you provided on the form. Insufficient vaccinations or incomplete records are the main reason a form is not approved on the first attempt.

Q. Should I upload the form now if I have outstanding vaccinations to complete?
A. No. If you are in the process of completing required vaccinations, please wait to upload your form into eTrieve until the vaccination process has been completed. We cannot approve future vaccine dates.

Q. What will happen if I don’t submit my immunization record or it hasn’t been fully approved?

  1. You will not be able to move on to campus if you are residential;
  2. You will not be able to attend classes;
  3. Booster shots or blood tests may be required in the event you cannot find your complete records. Please talk to your provider or local health department about your options.

Q. I would like to request an exemption from a vaccination or vaccinations. How do I do this?
A. If you would like to request exemption from a vaccination, there is a place to indicate this on page 2 of the Health Immunization Form.

  1. You will need to write the word “Exempt” next to the individual vaccine/vaccines you are claiming exemption from. You need to sign and date the bottom of the form under Statement of Exemption or your form will be rejected.
  2. By requesting exemption, you acknowledge that in the event of an outbreak of vaccine preventable communicable disease, you may be asked to quarantine off campus for safety and transmission mitigation efforts.
  3. If you decide to receive vaccinations that you previously requested exemption from, please upload proof of vaccination to eTrieve so we may update your status.
